The treadmill VO2max test is the criterion measure of aerobic power for runners. Here is a treadmill VO2max test conducted by a guy named Joseph, reaching a score of 64 ml/kg/min (5.3 L/min). You can see that quite a significant gradient is being used, and it gets pretty tough at the end.

Example of a seated arm-crank VO2 max test, performed by a quadriplegic. As you can see, the VO2max test does not always have to be performed on a treadmill, any exercise mode can be used, and the most specific to the athlete and his sport should be used if possible.
